Prescription Refill and Food Order Request Form

Please use the form below to request your prescription refill or food item.

**Please read the following instructions below carefully. This will save you time when picking up your order and ensure that your pet receives their medication on time:

IMPORTANT – Prescription Refills and Food Orders are not confirmed until you have received notification. A staff member will contact you by email to confirm that we are processing your request or to provide special information regarding your pet’s medication. We ask that you refrain from coming to the clinic until you have received confirmation, via phone or email, that your order is ready to be picked up.

Please allow 24 hours for order processing.

PLEASE NOTE – We understand that sometimes time escapes us and you may forget to renew medications before they are about to run out. For such situations, we ask that you kindly call the clinic directly and we will do our best to help.

Please understand that some medications are specially ordered from a pharmacy and require additional days to ship.

Next time you are in to pick up your pet’s medication, please ask one of our Client Service Team members if your medication requires a special order.

  • Many prescriptions require your pet to be examined on a routine basis before dispensing. Although your pet’s medication makes them feel better or they’ve been on the same dosage for a long time, some medications have severe adverse effects when used long-term. For this reason we require an up to date (annual) general health exam +/- blood work – NO EXCEPTIONS! This ensures that your pet is healthy enough to handle the potential side effects of some prescriptions and provides further confirmation that the medication is appropriate for your pet’s current condition.
